About the Role

We are seeking a highly skilled and compassionate Qualified Social Worker to join our Adults Locality Team on a permanent basis. As a key member of our team, you will play a crucial role in supporting individuals to live independently and with dignity.

Key Responsibilities
  • Conduct comprehensive assessments of service users' needs to identify their requirements and develop personalized care plans.
  • Work collaboratively with other professionals to ensure the best outcomes for our service users, including healthcare professionals, family members, and carers.
  • Provide support and advocacy for service users, promoting their independence and well-being.
  • Maintain accurate and up-to-date records, ensuring compliance with relevant legislation and policies.
Requirements
  • Degree in Social Work and registration with Social Work England.
  • Proven experience in adult social care, with a strong understanding of assessment and care planning principles.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a commitment to continuous professional development.
