Make a Difference in City & Hackney Residents' Health and Wellbeing

This is an exciting opportunity to be part of a pioneering Social Prescribing Service, commissioned by City & Hackney CCG, which has been recognized for its excellence in delivering personalized support to individuals. As a Wellbeing Coordinator, you will work as part of a Primary Care Network Multi-Disciplinary Healthcare Team, providing holistic and strength-based support to individuals, helping them identify and address the wider issues impacting their health and wellbeing.

You will have the opportunity to forge strong links with local VCSE organizations, community, and neighbourhood networks, building on existing resources to create a menu of community groups and assets to connect individuals to as part of their wellbeing plan. Your excellent interpersonal and communication skills will enable you to build trusting relationships, empowering individuals to take control of their wellbeing.

This role requires a good listener with experience of working positively with individuals facing complex social and emotional challenges. Being creative and solution-focused, you will remove barriers to health and wellbeing, making a tangible difference in the lives of City & Hackney residents.
