About the Role
We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our established Multidisciplinary Team within the Community Mental Health setting. As a key member of our team, you will provide holistic care to individuals experiencing mental health issues and support their recovery journey.
Key Responsibilities
- Work as part of an established Multi-Disciplinary Team to provide comprehensive assessments and formulate personalized care plans that reflect the needs of service users, their families, and wider support networks.
- Act as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, providing psycho-social interventions, education, and support to prevent relapse and admission.
- Support carers and system partners, undertaking Mental Health Nursing Needs Assessments/Care Act Assessments as required.
- Administer prescribed depot injections/medication monitoring and Clozapine Clinic as required.
- Conduct risk assessments of individuals and potential hazards in their environment.
