About the Role

Nations Recruitment is seeking an experienced Social Worker to join our team in a Case Management role. As a Social Worker, you will be responsible for undertaking longer-term, complex social work interventions with older adults and physically disabled individuals.

Key Responsibilities
  • Conduct assessments and interventions with complex caseloads, including those with concurrent mental health and substance misuse needs.
  • Undertake Safeguarding enquiries, assessments, and unscheduled reviews under the Care Act 2014.
  • Work collaboratively with multi-disciplinary teams to provide Continuing Health Care processes.
  • Develop and implement individualized care plans to meet the needs of service users.
  • Engage in duty rotation and participate in the team's operational framework.
Requirements
  • 2 years' post-qualifying statutory experience in social work.
  • Experience working with adults with complex needs.
  • Strong knowledge of Safeguarding procedures and mental capacity legislation.
  • Ability to work collaboratively as part of a team.
  • Excellent communication and interpersonal skills.
