Key Responsibilities- Conduct assessments and interventions with complex caseloads, including those with concurrent mental health and substance misuse needs.
- Undertake Safeguarding enquiries, assessments, and unscheduled reviews under the Care Act 2014.
- Work collaboratively with multi-disciplinary teams to provide Continuing Health Care processes.
- Develop and implement individualized care plans to meet the needs of service users.
- Engage in duty rotation and participate in the team's operational framework.
- 2 years' post-qualifying statutory experience in social work.
- Experience working with adults with complex needs.
- Strong knowledge of Safeguarding procedures and mental capacity legislation.
- Ability to work collaboratively as part of a team.
- Excellent communication and interpersonal skills.
- A competitive hourly rate.
- Flexible working arrangements with a 50/50 home/office split.
- Use of a Team Oyster card for visits.
- Supportive and responsive management structure.
- Opportunities for transition to permanent contract and further qualifications.
