Job Description- Provide person-centred care and support to individuals with complex needs
- Develop and implement individualised support plans
- Work collaboratively with colleagues to achieve positive outcomes
- Minimum 12 months experience in Adult or Children's support settings
- Excellent written and verbal communication skills
- Able to build rapport and establish strong relationships
- Experience of individual support planning
- Valid UK driving licence and access to a vehicle
- Weekly pay with Bank Holiday premiums
- Holiday pay accrued or added to pay rate
