Job Title: Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team. As a Support Worker, you will play a vital role in supporting individuals with their daily living tasks, promoting their independence, and enhancing their overall well-being.

Key Responsibilities:
  • Undertake daily living tasks as agreed with the service user, their family, and the professionals involved with the individual service plan.
  • Work within the policies and procedures of Connected Living, ensuring the safety and well-being of service users.
  • Complete Health and Safety checks within the service, in line with the policy and procedures in place.
  • Ensure safe medication is delivered and ordered to the service users' homes.
  • Provide service users with opportunities to express their preference as to the way tasks are carried out.
  • Maintain and promote choice and independence to the service users we work with.
  • Manage service users' behaviours in the least restrictive way possible.
  • Ensure all daily notes, handovers, and any relevant documentation is completed by the end of shift, and high-quality efficient handovers are provided to the oncoming shift.
  • Work as part of a team and wider MDT to support the clients' emotional and physical well-being.
  • Develop and maintain professional working relationships with service users and the relevant multi-disciplinary team to ensure positive outcomes for the service user.
