We're seeking a highly skilled and compassionate Complex Case Manager to join our team at Routes Healthcare. As a key member of our care team, you will be responsible for managing complex care packages and providing exceptional support to our clients.

Your core role will include:

  • Coordinating and leading the transition, start-up, and continuity of all complex care packages
  • Managing the clinical training needs of the care team and maintaining an appropriate skill mix to ensure safe and effective clinical practice
  • Developing comprehensive care plans and risk assessments
  • Providing support for care packages through observations and mentoring sessions, and regularly reviewing

As a Complex Case Manager, you will be a responsive advocate for complaints and safeguarding, and will encourage improvement. You will also be responsible for upholding high standards in care and prioritizing a person-centered approach.

We're looking for someone who is passionate about putting people first, has excellent organizational and time management skills, and has a good understanding of CQC requirements. If you have experience in managing complex care packages with clinical interventions, including spinal injuries, acquired brain injuries, motor neuron disease, and multiple sclerosis, we'd love to hear from you.
