About the Role

We are seeking a skilled and compassionate Community Health Care Assistant to join our team at Kent Community Health NHS Foundation Trust. As a Community Health Care Assistant, you will play a vital role in providing high-quality patient care and support to individuals in their own homes and in residential care settings.

Key Responsibilities
  • Provide person-centered care to patients, focusing on their physical, emotional, and social needs.
  • Work collaboratively with multidisciplinary teams to develop and implement care plans that meet the unique needs of each patient.
  • Monitor patient progress, identify areas for improvement, and make recommendations for enhanced care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date with best practices and advancements in community health care.
Requirements
  • Relevant level 3 qualification (NVQ/ QCF Diploma or certificate of care / Advanced apprenticeship) or equivalent or working towards.
  • Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C or equivalent in English and Maths).
  • Minimum of 2 years' experience as a Healthcare Assistant/ Worker in a health or social care setting.
  • Experience of working as part of a team.
  • Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ CIS/RIO).
Desirable Criteria
  • Experience of community nursing environment.
