Care Pathway Coordinator Job Opportunity

Barts Health NHS is seeking a skilled Care Pathway Coordinator for Older Adults to join our team. As a key member of our dementia team, you will play a crucial role in improving the care and experience of patients with dementia and delirium.

About the Role:

  • You will work collaboratively with our dementia teams to develop and implement effective care pathways for patients with dementia and delirium.
  • Responsibilities will include providing specialist advice and support to ward teams, patients, and their carers.
  • You will also participate in the delivery of our Trust Dementia Education Programme, ensuring that staff receive high-quality training on dementia care.

Requirements:

  • Registered Nurse - General or Mental Health or Occupational Therapist qualification.
  • A minimum of 2 years' relevant experience at Band 5 or above.
  • Experience of caring for people with dementia in the acute setting.
