Older Adults Care Coordinator

**Job Description:**
We are currently seeking an experienced Older Adults Care Coordinator to join our team in Bury. This is an excellent opportunity for a qualified professional to work within a community mental health setting.
The ideal candidate will have prior experience of working in A&E or community services and be responsible for conducting mental health assessments, making placement recommendations, and liaising with a multidisciplinary team (MDT).
This is a full-time position, offering a competitive salary of £37.00 per hour, depending on experience. The applicant must be a Qualified professional registered with their respective body (NMC, HCPC, or Social Work England).
Responsibilities:
- Carry out comprehensive mental health assessments
- Make placement recommendations
- Liaise with MDT
Requirements:
