As a Youth Mental Health Transition Support Specialist, you will be part of the Betsi Cadwaladr University Health Board team, working closely with CAMHS and adult mental health services to ensure seamless transitions that promote resilience and recovery.

The Role

You will support multidisciplinary teams in delivering holistic care to children, young people, and families accessing community child and adolescent mental health services. With a focus on older adolescents transitioning between services, your work will be instrumental in ensuring person-centred and needs-based transitions.

Key Responsibilities
  1. Collaborate with CAMHS and adult mental health services to develop effective transition plans.
  2. Support the delivery of high-quality day-to-day care, prioritising the needs of each individual.
  3. Develop positive relationships with host teams, departments, and partner agencies to facilitate smooth transitions.
Requirements
  • NVQ Level 3 or equivalent experience; willingness to undertake NVQ level 3 within 2 years.
  • Knowledge of mental health principles and practices.
  • Experience working with people, preferably in mental health settings.
