Job Description:

We are seeking a compassionate and dedicated Support Worker to join our team at The Wilf Ward Family Trust. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with profound learning disabilities and complex physical health needs.

Key Responsibilities:

  • Provide personal care and support to individuals with complex needs, including assistance with daily living activities, mobility, and medication management.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders to ensure a person-centered approach to care.
  • Work collaboratively with the multidisciplinary team to ensure seamless care delivery and effective communication.
  • Participate in training and development opportunities to enhance your skills and knowledge in areas such as manual handling, hoisting, and medication administration.
  • Contribute to the creation and implementation of individualized care plans, ensuring that the needs and preferences of each person are respected and met.

Requirements:

  • Relevant qualifications and experience in care and support work.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Physical ability to perform manual handling tasks and work in a fast-paced environment.
  • Full UK driving license (essential for this role).
