Job Description:
We are seeking a compassionate and dedicated Support Worker to join our team at The Wilf Ward Family Trust. As a Support Worker, you will play a vital role in providing high-quality care and support to individuals with profound learning disabilities and complex physical health needs.
Key Responsibilities:
- Provide personal care and support to individuals with complex needs, including assistance with daily living activities, mobility, and medication management.
- Develop and maintain positive relationships with individuals, their families, and other stakeholders to ensure a person-centered approach to care.
- Work collaboratively with the multidisciplinary team to ensure seamless care delivery and effective communication.
- Participate in training and development opportunities to enhance your skills and knowledge in areas such as manual handling, hoisting, and medication administration.
- Contribute to the creation and implementation of individualized care plans, ensuring that the needs and preferences of each person are respected and met.
Requirements:
- Relevant qualifications and experience in care and support work.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Physical ability to perform manual handling tasks and work in a fast-paced environment.
- Full UK driving license (essential for this role).
