Job Summary

This is an exciting opportunity to join our team as a Learning Disability Nurse Specialist - Commissioning. As a key member of our multidisciplinary team, you will play a vital role in ensuring that patients receive high-quality, person-centered care.

Your primary responsibility will be to assess the needs of adults with learning disabilities and complex health requirements, and commission packages of care that meet their individual needs. You will work closely with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.

In addition to your clinical expertise, you will also be responsible for supporting the development of policies and procedures related to Continuing Healthcare, Funded Nursing Care, and Section 117 aftercare.

We offer a competitive salary of £45,000 - £65,000 per annum, depending on experience, and a range of benefits, including flexible working arrangements and professional development opportunities.
