The successful candidate will work in a person-centred way, understanding positive behaviour support and adapting their approach to meet the individual's unique needs. You will work closely with key stakeholders, advocates, and other professionals to ensure the individual receives the best possible care.

Key Responsibilities
  • Support the individual with complex needs in their own home, promoting independence and well-being.

  • Collaborate with key stakeholders, advocates, and other professionals to ensure the individual receives the best possible care.
  • Develop and maintain a deep understanding of the individual's needs, preferences, and goals.
