About the Role

We are seeking an experienced and skilled Advanced Practitioner to join our Hospital at Home Frailty team. As a key member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our service.

Key Responsibilities
  • Provide rapid assessment, diagnostics, and treatment to individuals within the Hospital at Home service, using advanced clinical skills and judgment.
  • Work collaboratively with senior medical colleagues and other healthcare professionals to ensure timely and effective management of patient care.
  • Contribute to service development and policy implementation, informed by current best practice and evidence-based research.
  • Lead and mentor junior staff, providing guidance and support to ensure high-quality patient care.
Requirements
  • Professional registration with either the NMC or HCPC.
  • Evidence of MSc-level education and relevant clinical experience.
  • Leadership and management skills, with experience of teaching, mentorship, and supervision.
  • Ability to interpret and apply national and international guidelines to local protocols and practice.
  • Effective communication and interpersonal skills, with the ability to work collaboratively with multidisciplinary teams.
