Job Title: Adult Clinical Lead

We are seeking an experienced and skilled Adult Clinical Lead to join our team at NELFT NHS Foundation Trust. As a key member of our community healthcare team, you will be responsible for managing a caseload of patients with complex health and social care needs.

Key Responsibilities:

  • Manage a caseload of patients with complex health and social care needs, providing case management and facilitating early discharge from hospital where appropriate.
  • Work autonomously to case manage patients, using advanced nursing skills such as assessment and prescribing to manage their conditions in the community.
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care coordination and delivery.
  • Develop and implement quality improvement initiatives to enhance patient outcomes and service delivery.
  • Provide education, training, and development opportunities for colleagues to support their professional growth and development.

About the Role:

Requirements:

  • Registered Nurse with at least 2 years of community experience managing a complex caseload of patients.
  • Advanced nursing skills, including assessment and prescribing.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to develop and implement quality improvement initiatives.
