Job Summary
We are seeking a qualified Senior Discharge Facilitator to join our team at Berkshire Healthcare NHS Foundation Trust. The successful candidate will work closely with our multidisciplinary team to ensure the safe and timely discharge of patients from the hospital to the community.
Main Responsibilities
The Senior Discharge Facilitator will be responsible for:
- Working closely with hospital staff, community teams, and patients to identify and address discharge barriers.
- Developing and implementing discharge plans that meet the individual needs of patients.
- Collaborating with multidisciplinary teams to ensure seamless transitions of care.
- Providing education and support to patients and their families to promote independence and self-care.
- Monitoring and evaluating discharge outcomes to inform quality improvement initiatives.
