Berkshire Healthcare NHS Foundation Trust is seeking a highly skilled and experienced professional to join our team as a Community Hospital In Reach Matron. This is a unique opportunity to work in a dynamic and innovative environment, providing specialist mental health and community health services to the people of Berkshire.
Main Duties of the Job
The successful candidate will work closely with the In Reach Team, Community Hospital Beds, Emergency Departments, Frailty Team, and Wards to reduce the stay of necessary hospital admissions by facilitating safe discharges of individuals living within West Berkshire. They will work autonomously to determine the needs of patients, referring them to appropriate services, including Primary care, Community services, social services, Mental Health, and Voluntary services, ensuring that a support network is in place on discharge.
Key Responsibilities
- Work closely with the In Reach Team to identify patients with developing complex health or social care needs.
- Develop and implement effective discharge plans to reduce hospital admissions.
- Collaborate with Hospital Based Teams to identify suitable patients for cluster meetings.
- Provide specialist nursing care to patients with long-term conditions.
- Work effectively with patients, families, and carers to promote independence and quality of life.
Requirements
- Post-graduate Diploma or equivalent in Specialist Practitioner qualification (RDN Degree, diploma, or Certificate in District Nursing).
- RGN with 1st Level degree.
- 5 years post-registration experience in the acute/community and preferable at minimum of 2 years as Band 6.
- Management qualification.
