Job Title: Adult Community Care Coordinator

Location: Bolton Area

Contract Type: Locum, 3-Month Assignment

Hours: Full-time

Estimated Salary: £30 per hour (assignment rate)

We are seeking a highly experienced Adult Community Care Coordinator to join our Adults Community Mental Health Team in the Bolton area. As a key member of our team, you will work with individuals aged 18 years and above who have severe and enduring mental health conditions.

Key Responsibilities:

  • Coordinate care pathways for service users, ensuring seamless transitions between services.
  • Deliver a recovery-focused, trauma-informed service to service users through comprehensive assessments and risk assessments.
  • Manage a complex caseload, including individuals on the Assertive Outreach pathway, requiring proactive and flexible working within a multi-disciplinary team model.

Requirements:

  • Degree or equivalent in social work
  • Current registration with Social Work England
  • Significant frontline experience in Social Services within the UK as a qualified Social Worker
  • Three years' references
  • Experience working in a CMHT setting
  • Proficiency in Paris or Liquid Logic systems
