About the Role

We are seeking a highly skilled and motivated Assistant Practitioner to join our Community Nursing Team in Carmarthenshire. As a key member of our team, you will play a vital role in providing high-quality care to patients in their own homes, working closely with experienced District Nurses and other healthcare professionals.

Key Responsibilities
  • Provide clinical care to patients, including wound management, administration of medication, and venepuncture.
  • Work collaboratively with the District Nurse Team Leader to manage a delegated caseload and ensure effective communication between healthcare professionals.
  • Develop and maintain strong relationships with patients, carers, and other agencies to ensure seamless care delivery.
  • Contribute to the development of care plans and risk assessments, ensuring patient safety and well-being.
  • Participate in ongoing professional development to maintain and enhance clinical skills and knowledge.
Requirements
  • Level 4 vocational qualification in a relevant healthcare subject or equivalent.
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
  • Experience of providing clinical care in a community or primary care setting.
  • Strong interpersonal and communication skills, with the ability to manage challenging situations.
  • Ability to work independently and as part of a team, with a flexible approach to working hours.
