About the Role

Hywel Dda University Health Board is seeking a skilled and dedicated Band 4 Assistant Practitioner to join our Community Nursing Team. As a key member of our team, you will play a vital role in providing high-quality care to patients in their own homes, working closely with experienced District Nurse Team Leaders and other healthcare professionals.

Key Responsibilities

  • Support the management of a delegated caseload, providing complex care management to patients in their own homes.
  • Ensure high standards of care are delivered to patients and their families/carers within the home or clinic environment.
  • Liaise with staff, patient/carers, and other agencies/organisations to ensure appropriate delegation of duties and risk management.
  • Communicate effectively with those involved in the care package, including patients, families, and healthcare professionals.

Requirements

  • Level 4 vocational qualification in a relevant healthcare-associated subject or Certificate in Healthcare Support Worker Education.
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
  • Evidence of continuing personal development relevant to the role in healthcare.
  • Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
  • Clinical knowledge relevant to the role.
  • Demonstrates understanding of the importance of following procedures and treatment plans.
  • Demonstrates an understanding of the legislation related to Safeguarding adults/children, The Mental Capacity Act, and Deprivation of Liberties Safeguards to keep adults within their care safe.

Desirable Qualifications and Experience

  • Local knowledge of statutory and voluntary services.
  • Teaching or assessment qualification.
  • Experience in a range of clinical skills, including palliative care.
  • Experience of providing care in a community or primary care setting.
  • Administration of medicines via oral, topical, SC, IM routes.
  • Skills in non-complex wound care.

Language Skills

  • Welsh Speaker (Level 1) is desirable.
