Job Title: Care Specialist

The Sefton Carers Centre is seeking a highly motivated and empathetic Care Specialist to join our team. As a key member of our staff, you will be responsible for providing excellent support and assessment services to carers in the Sefton area.

About the Role:
You will work closely with our Carers Support & Safeguarding Lead to provide high-quality assessments, identify carer needs, and develop tailored support plans. Your exceptional communication skills will enable you to build strong relationships with carers, ensuring their voices are heard and their needs are met.

Key Responsibilities:
• Conduct thorough assessments of carer needs and develop individualized support plans
• Provide regular reviews and updates to carers, adjusting support plans as needed
• Collaborate with internal teams and external agencies to ensure seamless delivery of care services
• Maintain accurate records and reports, adhering to data protection guidelines

Requirements:
To succeed in this role, you must possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A degree in Social Work or a related field is desirable, along with experience working with vulnerable adults and carers. You should also have a basic understanding of safeguarding policies and procedures.
