We are seeking dedicated and compassionate Reablement Support Workers to join our expanding Discharge to Assess (D2A) Team. This innovative team provides short-term care and reablement services directly in patients' homes, ensuring a smooth transition from hospital to home.
Your Role:
- Provide personalized support to patients recovering from illness or injury.
- Assist with daily living activities, promoting independence and well-being.
- Work collaboratively with healthcare professionals to ensure a seamless care journey.
