**Reablement Support Worker Opportunity**
Join Candlelight Care in providing a short-term care and reablement service to patients in their own homes, bridging the gap between hospital and home.
This role is part of our expanding DISCHARGE TO ASSESS (D2A) Team, offering a rare opportunity to make a difference in people's lives.
We provide a supportive and collaborative environment, with a focus on delivering high-quality care and reablement services.
**Key Responsibilities:**
- Deliver high-quality care and reablement services to patients in their own homes
- Work collaboratively with the D2A Team to ensure smooth transitions between hospital and home
- Provide support and guidance to patients and their families
**Requirements:**
- Previous care experience is desirable but not essential
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
