Rehabilitation Assistant

We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our Community Stroke Team at Northampton General Hospital. As a key member of our team, you will play a vital role in providing goal-based reablement packages of care to individuals post-stroke, promoting independence and improving quality of life.
Key Responsibilities:- Provide rehabilitation and care to patients in their own homes or place of residence, following rehabilitation plans as directed by therapists or nurses.
- Assess and make recommendations for equipment, home adaptations, care provision, and ongoing rehabilitation, reporting these to therapists, registered nurses, or care managers.
- Deliver, fit, and demonstrate basic equipment to clients and appropriate others, ensuring client safety.
- Provide advice to patients, their families, and long-term carers, teaching and demonstrating the safe use of equipment or other techniques to optimize patients' mobility, functional ability, safety, and independence.
- Implement and record planned programs of care and rehabilitation to patients as directed by nurses, therapists, or care managers.
- Good level of basic education (GCSE or equivalent) in English and Maths.
- Study to NVQ level 3 or equivalent experience.
- Experience working face-to-face with members of the public.
- Experience working as a member of a team.
- Recent NHS experience.
- Experience working in a community health or social care setting.
- Experience of working with individuals with stroke.
- Basic knowledge and understanding of stroke.
- Basic knowledge and understanding of the role of occupational therapists, speech and language therapists, and physiotherapists in healthcare.
