Job Summary

The Northamptonshire Community Stroke Team is seeking a skilled and compassionate Rehabilitation Assistant to join their team. As a key member of the team, you will provide goal-based reablement packages of care to individuals post-stroke, promoting independence and optimal recovery.

Main Responsibilities
  • Deliver rehabilitation and care services to patients in their homes or place of residence, following rehabilitation plans as directed by therapists or nurses.
  • Assess and make recommendations for equipment, home adaptations, and ongoing rehabilitation, reporting to therapists, registered nurses, or care managers.
  • Provide advice and guidance to patients, their families, and long-term carers on the safe use of equipment and techniques to optimize mobility, functional ability, safety, and independence.
  • Implement and record planned programs of care and rehabilitation, using electronic notes, telephone, email, and virtual platforms.
  • Support patients in achieving identified treatment goals, involving them and their carers in the planning of care/rehabilitation programs.
Requirements
  • Good level of basic education (GCSE or equivalent) in English and Maths.
  • Study to NVQ level 3 or equivalent experience.
  • Experience working face-to-face with members of the public, as a member of a team, and in a community health or social care setting.
  • Basic knowledge and understanding of stroke and the role of occupational therapists, speech and language therapists, and physiotherapists in healthcare.
  • Awareness of risks to health and safety in the community.
