Job Title: Community Health Nurse

About the Role:

We are seeking a skilled and compassionate Community Health Nurse to join our team at Hounslow and Richmond Community Healthcare NHS Trust. As a Community Health Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, working closely with specialist nurses, allied health professionals, and social care colleagues to meet the needs of patients without unnecessary attendance in A&E or hospital admission.

Main Responsibilities:

* Provide skilled nursing care to housebound patients with complex needs
* Work independently as part of the Community Nursing Team covering a particular geographical area
* Conduct first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care
* Collaborate with multidisciplinary teams to ensure seamless care to patients

Requirements:

* Registered General Nurse with valid registration with the NMC
* Recent experience of nursing at Level 5 or evidence of a back to nursing programme
* Ability to converse articulately on current health issues and demonstrate knowledge in Essence of Care, Team Working, professional standards, and Adult Safeguarding
