We are seeking a highly motivated and enthusiastic individual to join our Personalised Care Team (PCT) as a Care Coordinator in North Halifax Primary Care Network (PCN). The successful candidate will play a vital role in delivering high-quality care and support to patients, working closely with the PCN team and other health and social care professionals.
The Care Coordinator will be responsible for bringing together all of a person's identified care and support needs and what matters to them, providing coordination and navigation for individuals and their carers across health and care services. They will build and maintain relationships within the Network, including staff at member practices, to encourage best practice and outcomes.
The ideal candidate will have excellent communication skills, a clear and polite telephone manner, and be competent in the use of MS Office and Outlook. They will also have effective time management skills, be able to follow policy and procedure, and be flexible and cooperative.
We are committed to providing a supportive and inclusive work environment, and the successful candidate will be required to undergo a satisfactory Disclosure Barring Service (DBS) check.
Key Responsibilities
- Undertake responsibilities in line with the PCN DES and focus on the delivery of the PCN care model to reflect local priorities, health inequalities, and population health management.
- Provide coordination and navigation for individuals and their carers across health and care services, working closely with the PCN team.
- Build and maintain relationships within the Network, including staff at member practices, to encourage best practice and outcomes.
- Work closely with other health and social care professionals to identify and support patients ensuring that access to appropriate services is available to them and their needs are met.
- Proactively identify and work with NH PCN patients to support their personalised care needs.
- Help people to manage their needs, answer their queries and support them to make and attend appointments.
- Ensure that people have good quality information to help them make choices about their care.
- Support people to understand their level of knowledge, skills, and confidence when engaging with their health and wellbeing.
- Signposting support to patients/family/care home on information about patient condition(s).
Requirements
- NVQ Level 2 in Health and Social Care or other relevant professional academic qualification.
- Excellent communication skills (written and oral).
- Clear, polite telephone manner.
- Competent in the use of MS Office and Outlook.
- Effective time management (planning and organising).
- Ability to follow policy and procedure.
- Flexible and cooperative.
- Flexibility to work outside core office hours.
- Satisfactory Disclosure Barring Service (DBS) check.
- General understanding of well-being and community services.
- Knowledge of local health and social organisations and the services they deliver.
- Driving licence and use of a car.
