Care Coordinator at a Primary Care Network

We are seeking a highly motivated and enthusiastic individual to join our team as a Childhood Immunisation Care Coordinator in a Primary Care Network. The successful candidate will play a crucial role in supporting our member practices to increase the uptake of childhood immunisations and promote NHS immunisation campaigns.
The Primary Care Network is a well-established network of 4 practices, serving a population of over 38,000 patients. The post holder will work on a rota basis within practices and at our PCN hub in the new purpose-built Mixenden Community Hub. The successful candidate will be required to travel independently between practice sites.
The ideal candidate will have excellent communication skills, be able to work autonomously, and have a flexible approach to change. They will be instrumental in the delivery of the practice/PCN's aims for patient-centred care.
NHS experience is not essential, as training will be provided. The successful candidate will be required to maintain accurate and contemporaneous patient records at all times.
Key Responsibilities:- Undertake responsibilities in line with practice demand for childhood vaccinations
- Provide education and advocacy about the benefits and safety of vaccinations, addressing any concerns or misconceptions individuals and their carers may have
- Work with practices to facilitate easy access to immunisation services
- Track immunisation rates and follow up with families
- Build and maintain relationships with patients and families, taking into account health inequalities and diversity
- Participate in outreach activities to raise childhood vaccination awareness and uptake
- Work closely with other health, social, and educational care professionals to identify and support patients, ensuring access to appropriate vaccinations and meeting their needs
