Mental Health Liaison Nurse

About the Role
We are seeking a highly skilled and experienced Mental Health Liaison Nurse to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our team, you will be responsible for providing high-quality care to patients with mental health needs, working closely with medical and nursing staff to ensure seamless care.
Key Responsibilities
- Conduct comprehensive psychosocial assessments of patients referred to the service, identifying their mental health needs and developing individualized care plans.
- Provide consultation-liaison services to clinical teams, assisting them in formulating appropriate plans of care and making decisions in cases where patients may be exhibiting complex needs and/or challenging behaviors.
- Initiate and participate in teaching and educational programs for all grades of nursing and non-nursing staff, promoting best practices in mental health care.
- Maintain a small caseload of patients, providing a broad range of treatments and clinical interventions as appropriate to individual needs.
- Participate in specific clinical projects, facilitating meetings, liaising, and negotiating with other staff as necessary.
- Promote a positive attitude and encourage a supportive culture in relation to mental health and psychiatric illness across the general hospital.
Requirements
- Registered Nurse (mental health) or Social Worker or Occupational Therapist with current professional registration.
- Experience of assessing, planning, and evaluating mental health nursing interventions.
- Experience of delivering psychiatric interventions to individuals who are acutely unwell.
- Evidence of professional updating and development in the nursing care of patients in an acute or community setting.
