Job Summary

We are seeking an experienced Advanced Clinical Practitioner to join our Urgent Community Response and Virtual Ward Team at Stockport Foundation Trust. As a key member of our multidisciplinary team, you will provide advanced clinical care and expertise to patients, working autonomously and as part of a team to deliver high-quality patient care.

Main Responsibilities
  1. Provide advanced clinical care and expertise to patients, including assessment, examination, investigation, and diagnosis.
  2. Work autonomously as an Advanced Clinical Practitioner, managing a caseload of patients and clinical priorities according to agreed protocols and working practices.
  3. Deliver individualized patient care, with direct responsibility for assessment, examination, investigation/diagnostic procedures, and diagnosis of patients with undifferentiated and undiagnosed presentations.
  4. Appropriately treat patients, resulting in the safe management and appropriate onward referral or discharge of patients at the end of the care episode.
  5. Participate in ward rounds, patient reviews, and multidisciplinary team meetings.
  6. Utilize scope of advanced practice to undertake Non-Medical Prescribing role and advise on medicine management issues associated with the patient group, working within Trust policy for Medicines Management.
Requirements
  • Professional Registration
  • Non-Medical prescribing course (in eligible professions)
  • MSc in clinical practice
Knowledge and Skills
  • Advanced clinical practice skills
  • Evidence of research and audit
  • Evidence of implementing change
  • Evidence of dissemination of knowledge in wider healthcare organisations
Experience
  • Post registration experience at a senior level (band 6 or above) in community or a relevant speciality area
  • Evidence of ongoing CPD
  • Evidence of teaching in a variety of environments
  • Ability to work independently.
  • Experience of using multidisciplinary approach to patient care
Working Arrangements

This is a permanent, full-time position. The salary is £53,755 to £60,504 per annum, pro rata. The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
