Job Title: Care Home Community Lead

We are seeking an experienced and ambitious individual to join our Community Team Leader group in our Planned Care community team in North Bradford localities.

The successful candidate will provide leadership and managerial supervision to a group of clinical practitioners working as a multidisciplinary team providing adult physical health services in the community.

This is a rewarding and challenging role where you will have the opportunity to develop new networks and gain additional knowledge and skills.

You will work with planned and unplanned District Nursing teams, and in addition, you will work alongside other key members of the primary health care team and other health and social care professionals to provide proactive safe care to our local population.
