Job Title: CPN - Home Treatment Team

We are seeking a skilled and dedicated CPN to join our Home Treatment Team in Greater Manchester. As a key member of our team, you will work closely with our professional Community Psychiatric Nurses to provide high-quality patient care.

Job Summary:

This is a fantastic opportunity for a hardworking and diligent CPN to join our team. You will be responsible for providing expert care to patients in their own homes, working closely with our multidisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide expert care to patients in their own homes
  • Work closely with our multidisciplinary team to ensure the best possible outcomes
  • Develop and implement individualized care plans
  • Conduct regular assessments and reviews
  • Collaborate with other healthcare professionals to ensure seamless care
Requirements:
  • Current and valid registration with the NMC
  • Proven experience within a CPN recovery role
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Commitment to providing high-quality patient care
