\Job Description
\We are seeking a Support Services Coordinator to join our team in London. As a Support Services Coordinator, you will be responsible for coordinating support services for our customers, helping them to achieve their goals and improve their overall well-being.
\Key Responsibilities
\- Coordinating support services for customers, including arranging appointments and meetings.
- Working with customers to identify and achieve their goals, using SMART goal planning techniques.
- Co-producing bespoke support plans with customers, involving key stakeholders as appropriate.
- Organising and carrying out regular planned reviews of support plans, or following an incident/significant change in a customer's circumstances.
Requirements
\- Experience of working with vulnerable adults, including those with complex needs.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues.
- Ability to work flexibly, including evenings and weekends.
