Job Description:

We are seeking a skilled and compassionate Care and Support Coordinator to join our team at Evolve Housing + Support. This role offers a unique opportunity to make a real difference in the lives of vulnerable young people.

About the Role:

The successful candidate will work closely with our customers, aged 16-24, who are currently experiencing homelessness. Our service is based in the Royal Borough of Kensington & Chelsea, where we provide individualized support to empower them to overcome their challenges and achieve their goals.

As a Care and Support Coordinator, you will play a vital role in developing life skills, promoting independent living, and connecting customers with resources and opportunities. Your expertise in safeguarding, housing, education, and employment will be invaluable in helping our customers navigate the complexities of their situations.

Key Responsibilities:

  • Support customers in developing life and independent living skills, including self-esteem building, relationship management, cooking, and budgeting
  • Facilitate access to education, training, or employment opportunities tailored to individual needs and goals
  • Navigate local agencies and specialist services to secure necessary support for customers
  • Maintain accurate records and reports on customer progress and outcomes

About You:

To succeed in this role, you will require:

  • Knowledge of safeguarding principles and practices
  • Understanding of homelessness causes and the challenges faced by young people
  • Experience working with vulnerable populations, particularly those affected by homelessness
  • Excellent communication and interpersonal skills
  • A person-centred approach to care and support
