We are seeking a skilled Rehabilitation Care Professional to join our team as a Case Manager in the North Yorkshire region. As a key member of our multidisciplinary team, you will work closely with children and young people who require intensive rehabilitation services following traumatic brain and spinal injuries.

The successful candidate will have a strong foundation in case management, excellent communication skills, and the ability to work effectively with families, caregivers, and healthcare professionals. You will be responsible for developing and implementing individualized care plans, conducting regular reviews and assessments, and ensuring that each child receives high-quality, person-centered care.

This role presents an excellent opportunity for a current Case Manager or an experienced Nurse, Physiotherapist, SALT, Occupational Therapist, or Social Worker to advance their career in case management and make a positive impact on the lives of those we serve.

Key qualifications include:

  • A degree in Occupational Therapy, Nursing, Social Work, Speech Therapy, or Physiotherapy
  • Experience working as a Clinical Case Manager or a strong background working with children who have backgrounds of brain injuries, spinal injuries, or birth trauma
  • Registration with the HCPC, NMC, or SWE
  • A UK driving license and own car
