Job Title: Advanced Learning Disability Physiotherapist

This is an exciting opportunity to work within the Calderdale and Huddersfield NHS Foundation Trust multi-disciplinary specialist respiratory team. The post holder will deliver pulmonary rehabilitation and self-management groups to patients with chronic respiratory conditions and long covid.

The team delivers a community pulmonary rehabilitation service that supports and promotes optimal self-care. This service may be delivered to a cohort in a class setting, individually tailored home-based rehabilitation as part of a supported discharge pathway, or virtually using digital and remote platforms.

Main Duties:
  1. To provide specialist assessment of patients with chronic respiratory disease including exercise testing and provide appropriate treatment to individuals in a variety of settings.
  2. To work within the community respiratory multidisciplinary team to promote self-management and hospital avoidance in patients with chronic respiratory disease.
  3. To provide ambulatory oxygen assessments to both chronic respiratory and long covid patients.
  4. To provide specialist multidisciplinary assessments of patients with suspected long covid syndrome and develop appropriate treatment plans to include exercise prescription.
  5. To be a member of the post covid multidisciplinary team meeting informing on other referrals required and rehabilitation suitability.
  6. To be the lead of a community-based pulmonary rehabilitation group.
  7. To be the co-lead of a community-based self-management programme for long covid with a specialism in exercise prescription.
Job Responsibilities:
  1. Act as a specialist physiotherapist within the respiratory and post covid service and contribute to evolving and embedding best practice into the clinical area.
  2. Manage own specialist physiotherapy caseload in both chronic respiratory disease and long Covid.
  3. Carry out specialist assessment and rehabilitation of patients with both respiratory disease and long covid syndrome. This will include implementing, evaluating and modifying individual treatment programmes.
  4. Possess a working knowledge of the community oxygen assessment service and complete ambulatory oxygen assessments if required.
  5. Develop effective links with other community and specialist respiratory services including General Practitioners, Practice Nurses and Respiratory Physicians, mental health services, and Community Pharmacists to provide specialist advice where needed and refer to other agencies as required.
  6. Take part in the education and training of Physiotherapists and qualified and non-qualified healthcare/social services/education staff including medical staff.
  7. Support the Clinical Lead in the day-to-day running of the pulmonary rehabilitation team including support staff.
  8. Ensure a high standard of patient care is provided including implementing and reviewing clinical standards relevant to the work undertaken.
Person Specification:

QUALIFICATIONS / TRAINING

Essential

  1. Physiotherapy Degree
  2. Registered with HCPC
  3. Clinical Educator training
  4. Computer literate with experience in using health-related systems such as Systmone, EPR and EMIS

Desirable

  1. Evidence of postgraduate training in the respiratory/long covid field
  2. Member of the Professional body
  3. Member of a respiratory network/ACPRC/long covid
  4. Manual handling key trainer

KNOWLEDGE, EXPERIENCE & EXPERTISE

Essential

  1. Recent experience demonstrating ability to assess, treat and evaluate care within the core area of both respiratory physiotherapy and long covid within the NHS
  2. Excellent clinical reasoning skills and the ability to maintain accurate records
  3. Significant previous experience in Pulmonary rehabilitation including Planning and delivering programs, and delivering group education sessions
  4. Demonstrable experience of working with patients within long covid service, including triage, multidisciplinary team assessment and rehabilitation both individuals and in a group environment.
