Lloyd Recruitment Services is seeking a skilled Patient Care Coordinator to join our dynamic healthcare team in Haywards Heath. As a key member of our team, you will play a vital role in enhancing the lives of our service users.
Key Responsibilities:- Collaborate with healthcare professionals to develop personalized care plans tailored to each patient's needs.
- Monitor and assess the quality of care provided to ensure it meets the highest standards.
- Organize training sessions for new staff and ongoing development programs for existing team members.
- Provide guidance and education to patients and their families about services and care procedures.
- Liaise with community resources and services to enhance patient care and access to additional support.
- Initiate and promote health and wellness activities within the community to improve overall patient well-being.
- Collect and analyze patient feedback to identify areas for improvement in service delivery.
- Act promptly to address urgent care issues and escalate matters as necessary to ensure patient safety.
- Maintain accurate records and documentation related to patient care and organizational activities.
- Full valid UK driving license (Essential)
- Background working within a similar position within the healthcare sector
- Level 3 qualification Health and Social Care (Preferred)
- Knowledge of systems used in the care sector
- Excellent communication and interpersonal skills
- Ability to build and maintain strong relationships
- Strong organizational skills and proactive problem solver
- Calm and reactive under pressure
- Ability to work both independently and as part of a team
