About the Role
We are seeking a highly motivated and experienced Physiotherapy Assistant Practitioner to join our Acute Medical Rehabilitation team at Hywel Dda University Health Board. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and rehabilitation services to patients across our medical wards.
Key Responsibilities
- Support and complete holistic patient assessments, considering rehabilitation needs and strategies for safe and timely discharge.
- Facilitate continuity of patient care and work collaboratively with other healthcare professionals to achieve optimal patient outcomes.
- Deliver rehabilitation programs to patients, including bed, chair, and standing activities, gait re-education, and gym-based exercise programs.
- Work independently and as part of a team to provide patient-centered care, with a focus on promoting independence and improving functional ability.
Requirements
- Good command of the English language and basic math skills.
- NVQ level 3 or equivalent qualification.
- Relevant additional courses to support clinical work.
- Significant knowledge and experience of the model of physiotherapy and ability to support therapeutic intervention with arms-length supervision.
- Broad range of experience as a support worker in a therapy service or associated field.
