About the Role

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our Integrated Community Stroke Team. As a key member of our team, you will play a vital role in supporting individuals with stroke-related needs, promoting independence, and enhancing their quality of life.

Key Responsibilities
  • Carry out rehabilitation plans as prescribed by registered therapists, ensuring adherence to instructions and recognized training.
  • Provide assistance to clinical staff in the assessment and treatment of patients, contributing to the rehabilitative process.
  • Follow a prescribed care plan to support service users at home, including assistance with activities of daily living tasks and provision of aids to support independent living.
  • Practice, reinforce, and progress activities of daily living, such as personal care, dressing, meal preparation, and other essential skills.
  • Participate in promoting healthy and independent living practices to patients, families, and carers.
  • Contribute to general administrative and non-clinical work, including photocopying, filing, liaison, and telephone calls.
  • Carry out safe and effective manual handling procedures to support service users in maintaining safety throughout interventions.
  • Be flexible to meet the demands of the service, working evenings, weekends, and bank holidays as required.
  • Participate in regular team meetings and attend other meetings as deemed necessary for the role.
