We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our Integrated Community Stroke Team. As a key member of our team, you will play a vital role in supporting individuals with stroke-related needs, promoting independence, and enhancing their overall quality of life.
Key Responsibilities- Carry out rehabilitation plans as prescribed by registered therapists, ensuring adherence to instructions and recognized training.
- Provide assistance to clinical staff in the assessment and treatment of patients, contributing to the rehabilitative process.
- Follow a prescribed care plan to support service users at home, including assistance with activities of daily living tasks and provision of aids to support independent living.
- Practice, reinforce, and progress activities of daily living, personal care, dressing, meal preparation, and other essential skills as appropriate.
- Participate in promoting healthy and independent living practices to patients, families, and carers.
- Contribute to general administrative and non-clinical work, ensuring the smooth functioning of the team, including photocopying, filing, liaison, and telephone calls.
- Carry out safe and effective manual handling procedures to support service users in maintaining safety throughout interventions.
- Be flexible to meet the demands of the service, working evenings, weekends, and bank holidays as required.
- Participate in regular team meetings and attend other meetings as deemed necessary for the role.
