We are seeking an experienced RGN Qualified Nurse to join our team as a Community Complex Care Nurse. The successful candidate will be responsible for managing a caseload of clients with complex needs in the community.

Key Responsibilities:

  • Provide high-quality care to clients with complex needs in the community
  • Manage a caseload of clients and provide regular updates to the team
  • Work collaboratively with the wider multidisciplinary team to ensure seamless care delivery
  • Attend meetings and make referrals as needed
  • Hold team meetings to discuss progress and address any challenges
  • Manage a team of non-clinical case managers to ensure effective care delivery

Requirements:

To succeed in this role, you will need to be a qualified RGN nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management. A car driver's license is also essential.
