We are seeking a Senior Nurse Practitioner - Community to join our client's CMHT team. The ideal candidate will have proven experience within a community recovery role and strong communication and interpersonal skills.

The Role

  • Participate in multi-disciplinary assessments of patients' health conditions
  • Formulate and implement care plans
  • Provide treatment as necessary
  • Use evidence-base practice and specialist skills to assess, plan, implement and evaluate interventions
