About the Role
We are seeking a skilled Primary Care Mental Health Practitioner to join our team at Coventry and Warwickshire Partnership Trust. As a key member of our Primary Care Network, you will play a vital role in supporting the mental health and wellbeing needs of local communities.
Key Responsibilities
- Review patients in primary care and assess their mental health needs, working closely with GP practices and other healthcare professionals to develop effective care plans.
- Offer brief interventions and provide dedicated support to individuals with mental health needs, including facilitating short-term case management and offering professional advice and guidance.
- Develop and maintain collaborative links with the third sector, providing clinical advice and support to Mental Health social prescribers and other professionals within the PCN multi-disciplinary team.
- Participate in pro-active mental health promotion and wellbeing initiatives, including World Suicide Prevention Day and World Mental Health Day.
- Support smooth transitions between primary and secondary care services, improving access to mental health services for service users and carers at the first point of contact.
