Job Title: Staff Nurse - Intermediate Care

We are seeking a dedicated and skilled Staff Nurse to join our Intermediate Care team at Bridgewater Community Healthcare NHS Foundation Trust.

About the Role

This is an exciting opportunity for a motivated and compassionate nurse to work in a dynamic and supportive environment. As a Staff Nurse in our Intermediate Care team, you will be responsible for providing high-quality care to patients who require rehabilitation and support.

You will work closely with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans, ensuring that each patient receives the best possible care and support.

Key Responsibilities
  • Provide high-quality nursing care to patients in our Intermediate Care facility
  • Liaise with the Intermediate Care Sister/Charge Nurse to ensure continuity of care and effective service delivery
  • Work collaboratively with other healthcare professionals and agencies to ensure effective service delivery
  • Perform comprehensive assessments of patient nursing needs and develop individualized care plans
  • Assess, plan, implement, and evaluate programs of care according to changing health needs
  • Maintain accurate records in accordance with NMC standards
  • Communicate and collaborate with other members of the Integrated Intermediate Care Team
Requirements
  • Current NMC registration
  • Able to develop effective interpersonal relationships across the health and social care setting
  • Wound Care Course
