About the Role:
As a Community Children's Health Care Professional, you will provide high-quality, personalised care to children and young people with acute or chronic health conditions. You will develop and implement tailored care plans in collaboration with families and healthcare professionals.
- Your expertise will be essential in educating families to promote child development and well-being.
- You will collaborate with multidisciplinary teams, including GPs, schools, social services, and specialists.
- Efficiently manage caseloads to ensure patient safety and timely care.
**Salary:** £31 per hour
