**Job Description:**

We are seeking an experienced staff nurse to join our community nursing team in North East Fife locality. As a community healthcare coordinator, you will work closely with patients, families, and healthcare professionals to coordinate care and support services.

**Key Responsibilities:**

- Coordinate care and support services for patients in their own homes

- Work collaboratively with other healthcare professionals to develop and implement individualized care plans

- Conduct regular assessments and reviews to ensure patients' needs are met

- Communicate effectively with patients, families, and colleagues to ensure seamless care delivery

- Participate in continuous learning and professional development to stay up-to-date with best practices

**Required Skills and Qualifications:**

- Registered Nurse qualification (RGN or RMN)

- Significant experience in community nursing or a related field

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Strong problem-solving and critical thinking skills
