**Job Overview:** Older Adult Community Nurse Specialist
About the Role:
We are seeking an experienced CPN to join our client's team in Central Lancashire. As an Older Adult Community Nurse Specialist, you will be responsible for delivering high-quality patient care and support to older adults.
Responsibilities:
- Conduct comprehensive assessments of patients' mental health status to inform personalized care plans.
- Develop and implement individualized care plans in collaboration with multidisciplinary teams.
- Provide emotional support and education to patients and their families, promoting mental wellness and recovery.
Requirements:
- Current and valid registration with the NMC.
- Proven experience within a community recovery role.
- Strong communication and interpersonal skills to build strong relationships with patients and colleagues.
What We Offer:
This is a 3-month contract position, requiring you to work Monday to Friday. The rate of pay is £24.50-£26 per hour, reflecting the high demand for skilled professionals in this field.
