Job Description

This is a specialist nursing role within the Central and North West London NHS Foundation Trust, providing health care services to individuals experiencing homelessness in the London Borough of Brent. The successful candidate will be responsible for delivering clinical interventions and promoting the overall health and wellbeing of service users.

About the Role

The Peripatetic Community Health Nurse Specialist will work closely with local hostels, general practitioners, and other healthcare services to provide outreach clinics, assessments, and referrals. This position requires strong communication and interpersonal skills, as well as the ability to work independently and prioritize tasks effectively.

The ideal candidate will possess post-registration experience in mental or general health, with a focus on substance misuse. A diploma in drugs and alcohol studies or equivalent qualification is desirable, along with training in cognitive behavioral therapy and motivational interviewing.

Key responsibilities include:

  • Providing specialist drug and alcohol outreach clinics in local hostels, GP surgeries, and organized health services.
  • Assessing clients' general health and providing early intervention treatment, signposting, and referrals to other healthcare services.
  • Developing and maintaining pathways into specialist services to ensure the health and wellbeing of residents are met.
  • Signing off service users to appropriate healthcare settings to meet their needs.
  • Serving as the single point of contact for primary care services, local hospitals, and other organized healthcare services.

**Essential Qualifications:** RMN/RN via the NMC professional body and evidence of continuing personal development and post-registration training in nursing care, assessment, and treatment of substance misuse clients.

**Desirable Qualifications:** Diploma in Drugs and Alcohol Studies or equivalent qualification, training in Cognitive Behavioural Therapy, and Completed Motivational Interviewing Training Module.
