Job Summary

An exciting opportunity has arisen for a Dementia Support Specialist to work on the Assessment Unit, part of the Emergency Care Directorate in a leading healthcare organization.

The successful candidate will work as part of a multi-disciplinary team to plan, organize, and facilitate a comprehensive range of individualized, group, and one-to-one activities for patients admitted to the unit.

Main Responsibilities
  • Develop and implement individualized care plans for patients with dementia
  • Collaborate with healthcare professionals to ensure high-quality patient care
  • Facilitate patient activities that promote social interaction, enhance self-esteem, and maintain or enhance functional ability
  • Provide education and support to patients, families, and caregivers
Requirements
  • Good standard of general education (NVQ Level 3 or equivalent experience)
  • Experience working in a caring environment, preferably with elderly patients
  • Excellent communication and interpersonal skills
  • Able to work autonomously and as part of a team
